Plan of Action:This health care system has 21 hospitals throughout the United States. All of their operations have a home care and/or a hospice program. This system tried for years to develop a strategy that assembled the talent and entrepreneurial mindset to compete in aggressive market places. Unfortunately with little success.
This health system was under growing pressure to address bundle payment systems, ACO and the better management of readmission rate and post-acute quality measures.
This health system found a partner to come in and manage the home care and hospice operations. They developed a JV and shared in risk and reward. The name of the company is Evolution Health. Evolution Health is a health care company that specializes in the management and development of post-acute care continuums.
After 8 months of discussion, a partnership was formed with an 18-month integration program roll-out agreement.
Outcome/Results:
Here are a few preliminary results after one-year:
- Six of their largest home care and hospice programs were integrated into Evolution.
- A strong expert management team was put in place.
- An improved model for serving the health system’s hospital and their challenges.
- Implementation of an EMR system.
- Volume has improved.
